Trying to find an expression for someone who should and wants do things but fails to do so

My friend is a great person, but on a regular base he fails to do what he should and even wants to do.
For example to call or visit his mother for her birthday.
To study for a test he already paid for and needs to set a date for since last year May in order to improve his chances for a better paid job.
To visit or call a sick friend.
To cook something for himself.
I hope that gives you an impression. It's across the board. As soon as he feels the slightest pressure on him, whether from outside or from within he seems to be unable to do what is needed. He always beats himself up over it afterwards too. Gets depressed.
I hope to find out if there is any help for him, if only I knew what term to google.
